            This is a very practical and probably one of the best books available for everyone who wants to understand the buzz words of Functional Safety.
            The Process Hazards Analysis techniques should be read/studied and understood by all process/instrumentation engineers and plant managers.
            That would be a good start and avoid industrial incidents like Bhopal, Piper Alpha, Texas City and unfortunately some more to follow...

                This paper describes the development of a management model to control barriers devised to prevent major hazard scenarios. Additionally, an audit technique is explained that assesses the quality of such a management system. The final purpose of the audit technique is to quantify those aspects of the management system that have a direct impact on the reliability and effectiveness of the barriers and, hence, the probability of the scenarios involved. First, an outline of the management model is given and its elements are explained. Then, the development of the audit technique is described. Because the audit technique uses actual major hazard scenarios and barriers within these as its focus, the technique achieves a concreteness and clarity that many other techniques often lack. However, this strength is also its limitation, since the full safety management system is not covered with the technique. Finally, some preliminary experiences obtained from several test sites are compiled and discussed.

                  Process hazard analysis (PHA) is widely used across a spectrum of industries and facilities, but there are few metrics for the evaluation and assessment of PHAs. Most existing protocols address PHA solely in terms of regulatory compliance, but do not address the completeness or depth of the assessment, as most PHAs have been performed on a ''performance basis'' under the relevant regulations. It is possible to objectively assess PHAs, in order to determine adequacy of completion and degree of review, using both audit protocol and scoring approaches. Audit protocol approaches offer simplicity and ease of use, and when combined with specific scoring for adequacy allow for informed decisions about remedial action with respect to the PHA.

                        The survey made within companies in the frame of the Leonardo da Vinci project ''Technological Training for SME's'' (Leonardo da Vinci project CZ/98/1/82530/PI/III.1.a/FPI ''Technological Training for SME's'' (1998-2001)[1]), English version. Ostrava: b.n., 2001 (coordinator A. Labodova)) documented the need for effective ways of integrating currently separate management systems for quality management, environmental management and health and safety. According to the evolution of such management systems, it is increasingly desirable and feasible to integrate these systems into one complex system for each company. Different approaches for integrating and gaining advantages from this integration are reviewed in this paper. Two ways of integration are discussed-firstly the introduction of individual systems followed by the integration of the originally separate systems, and secondly the development and implementation of an integrated management system, integrated from the very beginning. The procedure of a detailed methodology for the implementation of an occupational health and safety system, which was developed within research for Ph.D. thesis (Labodova A. The implementation of integrated management system. Ph.D. thesis, VSB-Technical University Ostrava, Czech Republic; 2002) based on an established risk assessment methodology (Risk Management. Leonardo da Vinci project no. CZ/98/1/82530/PI/III.1.a/FPI ''Technological Training for SME's'', English version. Ostrava: b.n., 2001, Module 3, Level 3 (project coordinator A. Labodova)), was used as a particular system approach and also for the implementation of integrated management system based on risk analysis on greenfield.
