Book Description

On average, a physician will interrupt a patient describing her symptoms within eighteen seconds. In that short time, many doctors decide on the likely diagnosis and best treatment. Often, decisions made this way are correct, but at crucial moments they can also be wrong -- with catastrophic consequences. In this myth-shattering book, Jerome Groopman pinpoints the forces and thought processes behind the decisions doctors make. Groopman explores why doctors err and shows when and how they can -- with our help -- avoid snap judgments, embrace uncertainty, communicate effectively, and deploy other skills that can profoundly impact our health. This book is the first to describe in detail the warning signs of erroneous medical thinking and reveal how new technologies may actually hinder accurate diagnoses. How Doctors Think offers direct, intelligent questions patients can ask their doctors to help them get back on track.

Groopman draws on a wealth of research, extensive interviews with some of the country's best doctors, and his own experiences as a doctor and as a patient. He has learned many of the lessons in this book the hard way, from his own mistakes and from errors his doctors made in treating his own debilitating medical problems.

How Doctors Think reveals a profound new view of twenty-first-century medical practice, giving doctors and patients the vital information they need to make better judgments together.

5 out of 5 stars If you want to be, go to, or are a doctor...read this.......2007-10-16

It was simply very interesting to read how a doctor navigated their own medical problems. To learn how many different opinions there were, and how someone who could actually weigh those differing opinions, considered their options.

Which presents the problem: how do we laymen weigh those differing opinions. Dr. Groopman presents solutions to that question, all the while giving you a fuller perspective on what the title promises.

There is also careful criticism of how medicine is conducted which is important for everyone involved in medicine to ponder.

5 out of 5 stars Everyone should read this.......2007-10-16

This is a good book that everyone should read. It will help them see how their doctors think and help identify potential flaws, which would lead to more effective disease treatment.

5 out of 5 stars A great look into the real world of medical care........2007-10-07

As an incoming first year medical student, I wanted to read a doctor's take on the behind-the-scenes work of medicine. What I got was an eye-opening and honest look at doctors, not as white-coated diagnosing machines, but as human beings with faults who are just trying to do their best. It is an excellent book both for those in medicine or who are going into medicine, as well as for laypeople who just want to get a better idea of where their doctor is coming from the next time they have an appointment.

5 out of 5 stars An Important Examination of the Problem Solving Process.......2007-10-02

I stumbled across this book in the library. My son has been sick for years, and I thought this might help. Dr. Groopman does an excellent job of helping the reader understand the process that doctors use to diagnose an illness, and thus how the more difficult cases are misdiagnosed. The case studies that he presents draw the reader in, so there are many short stories within the longer narrative. As an engineer I have seen the same mistakes made in solving complicated problems. I suspect a similar book could be written about solving difficult crimes, or problems in manufacturing. I directly applied the ideas in working with my son's doctors, and it was part of the solution to an accurate diagnosis and better treatment. I have mentioned the book to many friends and have now purchased two copies.

4 out of 5 stars Solutions at last?.......2007-09-30

The experience is common and can be more than frustrating. Something is wrong, the doctors don't listen (you are in and you are out and there was no time to ask questions) and they don't know what's wrong so say there is nothing wrong. This is the pathway to suicide.

Groopman does not give you the solutions but helps you find some. If this is not always a page turner, it is none-the-less easy reading that takes the layman into the doctors mind and shows how the doctor works. Current research is used, with a set of readable notes replacing the usual bibliography.

The layman and the doctor will both find the book full of really helpful hints.

As a person in health administration, I believe professionals would also find this book useful.

Book Description

Meta-analysis, decision analysis, and cost-effectiveness analysis are the cornerstones of evidence-based medicine. These related quantitative methods have become essential tools in the formulation of clinical and public policy based on the synthesis of evidence. All three methods are taught with increasing frequency in medical schools and schools of public health and in health policy courses at the undergraduate and graduate level. This book is a lucid introduction, and will serve the needs of students taking introductory courses that cover these topics. It will also be useful to clinicians and policymakers who need to understand the quantitative underpinnings of the methods in order to best apply the information that derives from them. The second edition of this popular book adds new material on cumulative meta-analysis as a method to explore heterogeneity. The coverage of cost-effectiveness analysis has been brought into close alignment with recommendations of the U.S. Public Health Panel on Cost-Effectiveness Analysis in Health and Medicine. Many of the examples have been replaced with more current examples, and all of the material has been updated to reflect recent advances in the methods and the emergence of consensus about some previously controversial issues. analysis. These three closely related methods have become even more important for synthesizing research since the first edition was published in 1994. And they have gained legitimacy as tools for guiding health policy.

5 out of 5 stars Best guide for analysis.......2006-12-20

There are several ways to do analysis of the health care industry but this book covers the big three. Cost Effectiveness is the most useful from a business standpoint. This book covers the math and theory behind each of these methods and gives strong arguments for how to write in each of them. As a health and pharmaceutical economist I found this book to be invaluable. It is written very clearly and helps sort through many of the issues especially meta analysis. If you are starting out in the health analyst field this is a must read and a book you will want to have handy at all times.

5 out of 5 stars An excellent introductory text.......2002-02-05

Meta-analysis, as both an applied and theoretical science, continues to develop from its rather humble beginnings. Since most physicians lack a strong background in quantitative science, rigorous statistical approaches to clinical decision making have been slow to gain widespread support. Despite its limitations, meta-analysis is proving to be an important vehicle for making sense of the increasingly overwhelming amount of published data in the biomedical sciences. Petitti's book goes a long way in demystifying meta-analysis for the rank and file and also serves as an excellent introduction to the field for the more statistically literate.

Most of my own work is in meta-analysis and I bought the book exclusively for this content and not information on decision analysis and cost-effectiveness analysis. I do not mean to minimize the importance of the latter but rather point out my own narrow interest in the Petitti text.

The book is well written and concise. Petitti "cuts to the chase" explaining the theory underlying meta-analytic methods and provides many useful examples from the medical literature. The text is easy to follow, understandable and well balanced. A thorough reading should leave the reader well prepared for further exploration of applied meta-analysis as well as capable of articulating its strengths, weaknesses and future direction.

Overall, Petitti's text is an excellent place to start for those interested in learning the fundamentals of "research synthesis". The book is also an extremely handy references for the more experienced practitioner. I highly recommend it.

5 out of 5 stars clearest yet description this topic.......1997-07-08

In a forest of mirky and arcane descriptions of a field that has come to dominate medical research and policy making, this book stands out for its clarity. Dr. Petitti speaks a language that normal people can understand. She avoids jargon and frequently uses quantitative examples that make it easy to plug in your own problems and directly apply her lessons to your tasks. This is easily the best book in print on this topic

Book Description

• A balanced, comprehensive guide to routine childhood vaccinations that offers parents the information they need to make the right choices for their child.

• Fairly examines the pros and cons of this highly charged issue.

Deciding whether or when to vaccinate a child is one of the most important--and most difficult--health-care decisions a parent will ever make. The recent increase in the number of vaccinations recommended and the concurrent controversies about whether vaccinations are safe or even effective have left many parents confused and concerned.

Midwife, herbalist, and mother of four, Aviva Jill Romm sifts through the spate of current research on vaccine safety and efficacy and offers a sensible, balanced discussion of the pros and cons of each routine childhood vaccination. She presents the full spectrum of options available to parents: full vaccination on a standardized or individualized schedule, selective vaccination, or no vaccinations at all. Negotiating daycare and school requirements, dealing with other parents, and traveling with an unvaccinated child are covered in detail. The book also suggests ways to strengthen children's immune systems and maintain optimal health and offers herbal and homeopathic remedies for childhood ailments. Emphasizing that no single approach is appropriate for every child, the author guides parents as they make the choices that are right for their child.

5 out of 5 stars Well-researched and informative.......2007-10-09

I am a former biochemistry researcher who researched experimental drugs that were submitted for FDA approval. I am also a new parent who recently had to face the decision of whether or not to vaccinate.

The FDA is made up of approvals boards that have had to retract or revise their decisions on many occasions. Science is still in its infancy in some regards, and when it comes to testing long-term effects of vaccines on infants and children, it is frankly primitive. As many readers know, the FDA has also been sued numerous times for conflicts of interest. Additionally, as a scientist, the possibility of diseases jumping the species barrier through vaccines via the DNA fragments of monkeys or aborted fetuses on which the vaccines were grown is a very real concern.

I am simply not convinced that vaccines have been sufficiently studied, each batch properly screened for the hundreds of known diseases that can be inadvertently transferred, and that all conflict of interests have been eliminated on the part of the FDA.

Of course, I realize the difficulties that such a task would pose. But that does not stop me from facing the fact that vaccines are still experimental substances with known adverse reactions, whose widespread use is heavily influenced by powerful pharmaceutical companies. These companies have a vested interest in making sure each and every newborn gets a Hepatitis B shot (regardless of whether a risk exists and regardless of the fact that the shot is ineffective within a matter of years) and that all children are given waves of booster shots, regardless of the concentration of the vaccine in the child's system at the time. Needless to say, my husband and I chose not to vaccinate.

I would encourage every parent to read this book and to understand their options. Research the history of conflicts of interest and errors on the part of the FDA. And above all, do not be afraid of asking questions and making a decision that others may find controversial. It is a shame that the Hippocratic Oath's edict to "First do no harm" cannot be faithfully applied to the subject of vaccines...

5 out of 5 stars Very informative!.......2007-09-05

I highly recommend this book to all new & potential parents. It is highly informative without swaying too much to either side of "to vaccinate or not". It gives the facts of each side and leaves the decision up to the parents.

5 out of 5 stars Good book to turn to for vaccine advice!.......2007-06-07

This book offered balanced pros and cons on vaccines, which is great when everything from doctors' offices is solely pro vaccine. I have read several books on vaccines is this was the most helpful.

Author: Now that the HiB vaccine has been around a little longer, can we have a Part II with more information on that?

5 out of 5 stars Finally, a balanced approach!.......2007-05-16

Finally we have an excellent book that gives the pros and cons of vaccination for parents and medical professions to consider. Too many books are biased either for or against. This author gives you ALL the facts from both perspectives, and lets the parent decide.

As an adult with hearing loss courtesy of a vax, I'm already anti-vax, but it's great to have this educational material on hand to educate doctors, teachers, and interfering family members.

3 out of 5 stars Not all that balanced.......2007-01-09

This book has some great information but isn't written in layman's terms. Therfore, portions of it can be difficult to understand for someone who isn't overly familiar with medicine. Also, the author definitely approaches the subject from the position that parents should not vaccinate, and I was hoping for a more balanced approach. I would recommend What Your Doctor May Not Tell You About Children's Vaccinations instead, which was written by a family practice doctor who began noticing adverse reactions following vaccinations. What Your Doctor... is much easier to understand and is written from a more neutral perspective.

5 out of 5 stars Family reading.......2007-06-09

This booklet is must reading for the elderly and members of their family. Presented in an easy to read format and style by a nursing home chaplain, it contains easy to understand medical information from a spiritual viewpoint. Don't wait for an illness to prompt you into reading this valuable booklet.

5 out of 5 stars Helpful for all ages.......2007-03-27

This little book is a must for all adult people. It addresses the serious question of what would you want done if you became mortally ill, were in an accident or in a coma and otherwise would not be able to speak for yourself. The frank, but gentle, theme of this book helps one to think about their own desires in these cases. One may think that it is only for "old people", but I think it is an important thing for each adult to consider carefully and let loved ones and your doctor know of your wishes. I would highly recommend this book for all adults.

This amazing little book, "Hard Choices for Loving People", is one of the best gifts you could ever give a patient and family who are facing these important end-of-life choices. Mr. Dunn says it so clearly, objectively, and compassionately that so much of the fear and pain of the decision is removed. I am a geriatric nursing leader of 43 years and have felt fortunate to have found this fine book in the early 1990s. I have shared it with many healthcare organizations and providers in California...from acute care to hospice, home health, Long-Term care, and now my church ! Thank you Mr. Dunn. I am a health care lawyer sometimes tasked with helping people face difficult end of life issues. This is the most sensitive and lovingly realsitic book of its kind that I've seen -- and there are many other good ones out there. Pssst -- you also can get it through a free pdf download at www.hardchoices.com . The booklet is prettier, but whatever the format, it's a vital read. I've always recommended it to clients, but my own family used it to open up hearts and minds -- and settle an ugly dispute between my mother and one of her brothers -- when my grandfather was dying of Alzheimer's. Hank Dunn's work is a God send.

Book Description

Decision making in health care means navigating through a complex and tangled web of diagnostic and therapeutic uncertainties, patient preferences and values, and costs. In addition, medical therapies may include side effects, surgery may lead to undesirable complications, and diagnostic technologies may produce inconclusive results. In many clinical and health policy decisions it is necessary to counterbalance benefits and risks, and to trade off competing objectives such as maximizing life expectancy vs optimizing quality of life vs minimizing the required resources. This textbook plots a clear course through these complex and conflicting variables. It clearly explains and illustrates tools for integrating quantitative evidence-based data and subjective outcome values in making clinical and health policy decisions. An accompanying CD-ROM features solutions to the exercises, PowerPoint® presentations of the illustrations, and sample models and tables.

5 out of 5 stars Concise, yet comprehensive sytematic approach to bioethics.......1999-10-22

Dr. Jonsen is one the nation's germinal thinkers in the field of bioethics. Through his experience and associations at several of the renowned medical academic institutions in this coutnry he sets forth a clear systematic approach to problems in bioethics. This includes a review of his "4-box" method for approaching ethical problems and then reviews basic ethical principles as well as legal definitions. His approach to clincal dilemmas provides a refreshing analytical process for dilineating issues and conflicts.

5 out of 5 stars This is an excellent resource for those in medical settings........1997-07-18

The authors suggest another method of evaluating ethical dilemmas. They suggest one examine the situation in light of medical indications, patient preferences, quality of life, and contextual features. Because the approach is clinical, rather than theoretical, some of the medical details presented are somewhat technical. This is a book for those working in a medical setting or otherwise engaged with medical ethics. I read the book based on the recommendation of a bioethics lecturer. It was further recommended recently by the Director of the Program in Medical Ethics for University of California, Irvine. --Mary Kay Buike, MSW, ACSW; Oncology Social Worker and member of a hospital Bioethics Committee

I bought all the books on cosmetic breast surgery, while researching my desire to have my breast lifted and enlarged, this book is by far the best, most informative and well- written. I also went on several consultations with different doctors and Dr. Freund was the most helpful and comforting physicians. His knowledge made me confident and the results were better than i hoped for. P.S. I travelled from North Carolina to see him and it was worth the trip.

My surgery went very well, and I am pleased with the outcome. Thanks for writing such a clear and informative book that answers the many questions women have concerning breast surgery.

Book Description

A unique, in-depth discussion of the uses and conduct of cost-effectiveness analyses (CEAs) as decision-making aids in the health and medical fields, this volume is the product of over two years of comprehensive research and deliberation by a multi-disciplinary panel of economists, ethicists, psychometricians, and clinicians. Exploring cost-effectiveness in the context of societal decision-making for resource allocation purposes, this volume proposes that analysts include a "reference-case" analysis in all CEAs designed to inform resource allocation and puts forth the most explicit set of guidelines (together with their rationale) ever defined on the conduct of CEAs. Important theoretical and practical issues encountered in measuring costs and effectiveness, evaluating outcomes, discounting, and dealing with uncertainty are examined in separate chapters. Additional chapters on framing and reporting of CEAs elucidate the purpose of the analysis and the effective communication of its findings. Cost-Effectiveness in Health and Medicine differs from the available literature in several key aspects. Most importantly, it represents a consensus on standard methods--a feature integral to a CEA, whose principal goal is to permit comparisons of the costs and health outcomes of alternative ways of improving health. The detailed level at which the discussion is offered is another major distinction of this book, since guidelines in journal literature and in CEA-related books tend to be rather general--to the extent that the analyst is left with little guidance on specific matters. The focused overview of the theoretical background underlying areas of controversy and of methodological alternatives, and, finally, the accessible writing style make this volume a top choice on the reading lists of analysts in medicine and public health who wish to improve practice and comparability of CEAs. The book will also appeal to decision-makers in government, managed care, and industry who wish to consider the uses and limitations of CEAs.

1 out of 5 stars Too many cooks (even if they are good ones) spoil the broth.......2006-02-24

This book pulls together a lot of information, but in my opinion it is written and organized very very poorly. I have over 10 yrs of experience in in the healthcare industry and came across this book during a grad level course in CEA.

Long, boring, convoluted sentences fill the pages of this book. It took about 3 times more time and effort to get through a page in this book compared to a page in respected journals like JAMA, NEJM, BMJ etc.

If you have lots of time and are doing research in this field - go for it. Alternatively if you want to quickly determine what CEA is about, I would recommend reading Peter Muennig and Kamran Khan "Designing and Conducting Cost-Effectiveness Analyses in Medicine and Health Care".

5 out of 5 stars That's a good textbook for cost-effectiveness research in m.......2000-09-20

Recently, there is less book to approach the cost-effectiveness point in medicine . I think this book can give us the new concepts of this research.

Book Description

Every year millions of Americans are diagnosed with cancer, stroke, HIV/AIDS, multiple sclerosis, ALS, and other life-threatening or life-altering diseases. When faced with a devastating diagnosis people must quickly understand the diagnosis, prognosis, and choose from several treatment options—while still in shock. AfterShock identifies the processes required to respond to a serious diagnosis, regardless of the specific disease.

Social psychologist Gruman offers practical advice in a 10-stage approach to making clear-headed, informed decisions about who to tell and when; how to get a second opinion or find the right doctor and hospital; where to find reliable online information and telephone help-lines about a particular diagnosis and treatment; how to navigate the health care maze; where to turn for support and comfort.

Dr. Gruman not only draws on her expertise in consumer health information and the advice of doctors and nurses, she shares the personal stories of people who have dealt with traumatic diagnoses, including her own experience of having had three different kinds of cancer and a dangerous heart condition. AfterShock is inspiring and empowering, and is an essential companion book to the disease-specific titles that most people with a devastating diagnosis purchase.

5 out of 5 stars Watch the Charlie Rose interview & then buy this book.......2007-07-13

Jessie Gruman, Ph.D., has written an essential reference for just about everyone. She offers a well-informed cushion and roadmap to support what most of us will face quite unprepared--the shock of learning that you or someone you love has a bad diagnosis. Gruman has been there--she's had 4 serious illnesses, including 3 different cancers. She has brought together her own personal experience and her extensive behavioral science and health policy professional knowledge in a very accessible reference. She was interviewed on the Charlie Rose Show on 7/11/07. (You can get the DVD through Amazon) Notice how engaged Charlie Rose becomes from the get-go in his interview with her, due to his own health issues and what she has to say. Don't wait until you get the bad news, and you are in shock. Watch this interview and then buy this book now.

5 out of 5 stars An essential book.......2007-02-17

This fabulous book should be in everyone's medicine chest because like bandages and aspirin, sooner or later, you're going to need it. Gruman skillfully and sensitively guides the reader through the full spectrum of concerns and activities that characterize the first several weeks after receiving a devastating medical diagnosis. Some of the chapters are devoted to topics you would expect in such a book: how to get a second opinion, how to manage the flood of emotions that spill over you. But others are entirely unexpected and therefore, especially welcome: how to deal with your insurance company, how to inform family members and co-workers, how to negotiate benefit managers, etc.

And just in case the above suggests that this is a dry and technical book, it's absolutely not. The great strength of AfterShock is that compelling and often heart-rending anecdotes from real patients, doctors, human resources specialists, and family members provide an emotional grounding for every bit of advice the book contains. This book is essential reading. Like it or not, we're all going to need it sooner or later.

The Bayesian approach involves synthesising data and judgement in order to reach conclusions about unknown quantities and make predictions. Bayesian methods have become increasingly popular in recent years, notably in medical research, and although there are a number of books on Bayesian analysis, few cover clinical trials and biostatistical applications in any detail. Bayesian Approaches to Clinical Trials and Health-Care Evaluation provides a valuable overview of this rapidly evolving field, including basic Bayesian ideas, prior distributions, clinical trials, observational studies, evidence synthesis and cost-effectiveness analysis.
Covers a broad array of essential topics, building from the basics to more advanced techniques.

Bayesian Approaches to Clinical Trials and Health-Care Evaluation is suitable for students and researchers in medical statistics, statisticians in the pharmaceutical industry, and anyone involved in conducting clinical trials and assessment of health-care technology.
